We are looking for an LVN with a dynamic personality to serve as Care Coordinator.  This LVN will work from our collaborating physician’s office and is responsible for establishing and promoting our progra, with the physician, staff and referring community; responsible for the overall productivity and profitability of the practice; insurance verification and entry of and obtaining demographics, incoming and outgoing activities for the program; preparing daily schedules, coordinating visits with caregivers and/or family members; handling orders, managing documents, care plan oversight (CPO), billing, making follow-up phone calls for weekly patient monitoring, ensuring patient’s receive information timely; and being the liaison with all referring and Practitioners agencies. This an hourly position with hours 8:00 AM to 5:00 PM, with health insurance, paid holidays, and PTO.

Ability to perform the functions of medication refills, labs, diagnostics, Document Management, triage calls, and ordering of equipment and supplies, etc.
Ability to articulate clearly and possess excellent command of oral and written skills.
Must possess excellent listening skills and the ability to ask probing questions, understand concerns and overcome objections.
Ability to be adaptable to a fast moving environment and use good judgment in all situations.
Ability to conduct problem analysis and problem solving.
