Nurse Practitioner or PA – Geriatric Primary Care, Office-Based Telehealth (Facilities Focus)

We are seeking a Nurse Practitioner or Physician Assistant to provide geriatric primary care via office-based telehealth. In this role, you will conduct primary care visits for geriatric patients in assisted living facilities via telehealth from our Brooklyn office, collaborating with on-site medical assistants and clinical liaisons to manage patient care, develop care plans, and document encounters in the eClinicalWorks (ECW) EMR system.

Our company brings healthcare directly to senior communities, providing primary care, specialist care, and care management to residents in assisted living and adult home communities. This position involves a predictable Monday–Friday schedule with no on-call requirements. You will have the autonomy to practice at the top of your license, with the support of a dedicated team, making a significant impact on the lives of our patients by providing consistent, high-quality care.

Job Type

Full-time, with a W2 or 1099 contractor option.

Compensation

- $155,000 base salary based on experience

- Earning potential up to $250,000 on a per-note structure

- 20 days of paid time off (PTO)

- Malpractice insurance coverage

- License/DEA fees covered

Schedule

Full-time, Monday–Friday. Flexible hours with a preferred schedule of 9am–5pm (8am–4pm or 10am–6pm options available). No call and no weekends.
