Nurse Practitioner or Physician Assistant — Patient Rounding (SNF) — $150,000-$160,000+
Confidential — National Post-Acute Care / PM&R-Led Clinical Services Group · Jacksonville, IL 62650
Job Description
We are seeking a Nurse Practitioner or Physician Assistant to join our mission-driven team. In this role, you will provide focused care in skilled nursing facilities, helping to reduce readmissions and improve recovery for patients during their most vulnerable moments — right after a hospital stay. This is a unique opportunity to enjoy a flexible schedule, strong team support, and the autonomy to manage your day without micromanagement. You will be the on-site clinical expert, making a tangible difference in the Jacksonville / Morgan County area of west-central Illinois.
Our company is a national post-acute care organization that champions value-based medicine across the healthcare continuum. We deliver physical medicine and rehab-led clinical services within skilled nursing facilities, partnering closely with therapy teams, primary care physicians, and facility staff. Our innovative model includes not only your on-site expert care but also a robust 90-day post-discharge follow-up program managed by our care coordination team, ensuring patients have continued support after leaving the facility.
This position involves a consistent but flexible weekday daytime schedule, typically Monday through Thursday, with the flexibility to manage your own hours as long as patient needs are met. You will enjoy exceptional job satisfaction stemming from a supportive, family-like culture that values your expertise, offers a true work-life balance with no nights, weekends, or call requirements, and provides the modern tools needed to complete documentation efficiently — often from the comfort of your home.
Key Responsibilities
- Round in assigned skilled nursing facilities on a consistent weekday daytime schedule, typically spending 4 hours per day on-site
- Oversee daily patient interactions through timely micro-visits that support focused assessments to evaluate recovery, manage pain, treat wounds, and address functional decline
- Collaborate closely with physical, occupational, and speech therapists, as well as primary care physicians and facility staff, to develop and adjust individualized recovery plans
- Monitor readmission risks, help close gaps in post-acute care, and participate in weekly Medicare meetings to discuss patient progress and barriers to discharge
- Utilize EHR system (Doc Now, integrated with PointClickCare) to complete accurate and timely documentation within 48-72 hours, often remotely
Qualifications
- Master's degree in Nursing and current certification as a Nurse Practitioner (NP), or graduation from an accredited Physician Assistant program with PA-C certification
- Active, unrestricted NP or PA license in the state of Illinois, with prescriptive authority
- Minimum of 2-3 years of experience in geriatrics, post-acute care, or a related field; experience in nursing facilities is strongly preferred
- Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions
- Excellent communication and time-management abilities to handle brief, efficient patient encounters
- Proficiency with EHR systems (PointClickCare experience a plus) and remote documentation
- Ability to work independently with a high level of autonomy and consistency
Benefits
Highly competitive compensation and benefits package totaling **$150,000-$160,000+**:
- Competitive base salary, typically $125,000–$135,000 annually, dependent on experience
- Monthly RVU bonus averaging $2,000+ per month ($25,000+ per year)
- Comprehensive health benefits package
- Paid time off to support your work-life balance
- Professional development opportunities and training in the post-acute / SNF rounding model
- Provision of remote charting tools and necessary supplies
Schedule
- Full-time, 40 hours per week
- Consistent weekday daytime hours only (typically Monday-Thursday)
- Flexible daily hours; manage your own time once rounding is complete
- No nights, no weekends, no holidays
- No on-call requirements
