PT / FT Primary Nurse Practitioner

This is an exceptional opportunity for a dedicated Nurse Practitioner to provide comprehensive primary care within a facility-based long-term care model in picturesque Conway, New Hampshire. You will be serving an elderly patient population, residents of long-term care facilities, managing their health needs in a consistent and familiar clinical environment that prioritizes continuity of care and patient well-being.

We are seeking an autonomous clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions and thrives as a collaborative team player, working closely with facility staff and attending physicians to ensure optimal patient outcomes.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with New Hampshire law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in New Hampshire (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
