You will serve an elderly patient population, residents of long-term care facilities, managing their wound care needs. We are seeking an autonomous clinician with a strong background in geriatric, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions, particularly those related to wound care, and thrives in a collaborative team setting. Your expertise will be crucial in providing high-quality, patient-centered care.

Key Responsibilities

- Provide primary/wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Wound care specific requirements: wound assessment, NPIAP staging, advanced wound products, wound vac experience, wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
