This is an exceptional opportunity for a dedicated Nurse Practitioner to join a facility-based long-term care model in scenic Conway, Arkansas. In this role, you will deliver facility-based Nurse Practitioner care within skilled nursing facilities, focusing on the elderly and long-term care residents. The day-to-day clinical environment involves managing a diverse patient panel, conducting rounds, addressing acute and chronic conditions, and working closely with facility staff to ensure continuity of care.

The ideal candidate is an autonomous clinician with a strong background in geriatric, adult, or family medicine. You should be comfortable managing complex chronic conditions and possess excellent clinical judgment. A collaborative team player who can effectively communicate with interdisciplinary teams and families is essential for success in this role.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Arkansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Arkansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
