This position involves delivering facility-based NP care within skilled nursing facilities, focusing on the elderly and long-term care resident population. You will operate in a collaborative clinical environment, working closely with facility staff and attending physicians to ensure seamless, high-quality care for a vulnerable patient demographic. We are seeking an autonomous and compassionate clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions and acute changes in condition within a long-term care setting. Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
