This is a unique opportunity for a dedicated Nurse Practitioner to provide comprehensive primary care within a facility-based long-term care model in Jonesboro, Arkansas. In this role, you will be integral to a facility-based Nurse Practitioner care model, serving as the primary care provider for long-term care residents within skilled nursing facilities. Your patient population will consist primarily of elderly individuals requiring ongoing medical management. The day-to-day clinical environment emphasizes proactive care, acute condition management, and collaborative efforts with facility staff to ensure optimal patient outcomes.

The ideal candidate is an autonomous clinician with a strong background in geriatric, adult, or family medicine. You should be comfortable managing complex chronic conditions and possess excellent clinical judgment. We are seeking a collaborative team player who can effectively communicate with nursing home staff, attending physicians, and patient families to coordinate holistic care plans.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Arkansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Arkansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
