This position involves delivering facility-based Nurse Practitioner care within skilled nursing facilities. You will be serving an elderly patient population, providing essential primary care services directly at their residence. We are seeking an autonomous clinician with a background in adult, family, or geriatric care. The ideal candidate is comfortable managing complex chronic conditions and thrives as a collaborative team player, dedicated to enhancing the quality of life for residents in a long-term care setting.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with OH law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in OH (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
