PRN Primary Nurse Practitioner

This is a unique opportunity for a dedicated Nurse Practitioner to provide essential primary care services within a facility-based long-term care model in Wauseon, Ohio. As a facility-based Nurse Practitioner, you will deliver comprehensive care to residents in skilled nursing facilities. The patient population primarily consists of elderly individuals with diverse and often complex long-term care needs. The day-to-day clinical environment involves managing chronic conditions, addressing acute changes, and collaborating with facility staff to ensure optimal patient outcomes.

We are seeking an autonomous clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions and thrives in a collaborative team setting, working closely with nursing home staff and attending physicians to develop and implement effective care plans.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
