PRN Primary Nurse Practitioner

Our innovative model focuses on delivering comprehensive Nurse Practitioner care directly within skilled nursing facilities. You will be serving an elderly patient population, residents of long-term care, managing their wound care needs in a dynamic clinical setting that prioritizes patient well-being and continuity of care.

We are seeking an autonomous clinician with a background in geriatric, adult, or family specialties, comfortable managing complex chronic conditions. The ideal candidate thrives in a collaborative team environment, working closely with facility staff and attending physicians to ensure optimal patient outcomes.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with PA law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in PA (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Expertise in wound assessment, NPIAP staging, and advanced wound products

- Experience with wound vac therapy

- Wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
