This position involves providing direct primary care services to residents within skilled nursing facilities. You will manage a diverse patient population, primarily elderly individuals with complex medical needs, within a collaborative and patient-centered clinical setting that prioritizes continuity of care and resident well-being.

The ideal candidate is an autonomous and compassionate clinician with a strong background in geriatric, adult, or family medicine. You should be comfortable managing complex chronic conditions, possess excellent clinical judgment, and thrive as a collaborative member of an interdisciplinary healthcare team.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with OH law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in OH (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
