Our innovative model focuses on providing high-quality, on-site primary care to residents in skilled nursing facilities. You will manage a diverse patient panel of elderly and long-term care individuals, addressing their complex health needs directly within their residential setting, fostering a consistent and patient-centered clinical experience.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family medicine. The ideal candidate thrives in managing complex chronic conditions, possesses strong diagnostic skills, and is a collaborative team player committed to interdisciplinary care.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with PA law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in PA (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
