This position focuses on delivering facility-based Nurse Practitioner care within skilled nursing facilities via telehealth. You will be serving an elderly patient population, residents in long-term care settings, managing their health needs remotely. The day-to-day clinical environment involves comprehensive virtual assessments, treatment planning, and collaboration with on-site staff to ensure seamless patient care, all while leveraging cutting-edge telehealth technology.

The ideal candidate is an autonomous clinician with a strong background in geriatric, adult, or family care, comfortable managing complex chronic conditions. You should be a collaborative team player, adept at communicating effectively with facility staff and other healthcare professionals to optimize patient outcomes. Your ability to provide empathetic and thorough care remotely will be key to success in this role.

Key Responsibilities

- Provide telehealth care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Telehealth/virtual care experience

- Multi-state licensure preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
