Our model centers on delivering expert Nurse Practitioner care directly within skilled nursing facilities, focusing on the unique needs of the elderly and long-term care resident population. You will manage a diverse caseload, providing essential wound care services in a collaborative and dynamic clinical setting, ensuring continuity and quality of care.

We are seeking an autonomous and compassionate clinician with a background in adult, family, or geriatric specialties. The ideal candidate is comfortable managing complex chronic conditions, particularly in wound care, and thrives as a collaborative team player, working closely with facility staff and attending physicians.

Key Responsibilities

- Provide primary/wound/telehealth care (as appropriate) to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with KS law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in KS (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Expertise in wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
