You will serve a vulnerable patient population of elderly and long-term care residents, managing their health needs in a dynamic day-to-day clinical environment that prioritizes continuity of care and proactive health management.

We are seeking an autonomous and compassionate clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions, possesses excellent clinical judgment, and thrives in a collaborative interdisciplinary team setting, working closely with facility staff and attending physicians.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
