As a facility-based Nurse Practitioner, you will deliver expert primary care services within skilled nursing facilities, serving elderly and long-term care residents. This model allows for a consistent patient panel and a collaborative clinical environment, ensuring continuity of care and strong relationships with both patients and facility staff.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions and thrives in a collaborative, interdisciplinary team setting, dedicated to providing high-quality patient-centered care.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Arkansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Arkansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
