This position involves delivering comprehensive, facility-based Nurse Practitioner care within skilled nursing facilities. You will serve a vulnerable patient population of elderly and long-term care residents, managing their complex health needs in a day-to-day clinical setting focused on continuity of care and improving quality of life.

We are seeking an autonomous clinician with a background in geriatric, adult, or family care, who is comfortable managing complex chronic conditions. The ideal candidate is a collaborative team player, eager to work alongside nursing home staff and attending physicians to ensure optimal patient outcomes.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience with wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
