This role involves delivering facility-based primary care services within skilled nursing facilities. You will be an integral part of a care team dedicated to the elderly and long-term care residents, managing their health needs in a stable and structured clinical environment. We are looking for an autonomous and compassionate clinician with a background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions and acute changes in status, thriving in a collaborative setting where teamwork with nursing home staff and attending physicians is paramount to patient success.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with OH law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in OH (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
