Our innovative business model focuses on delivering high-quality, facility-based Nurse Practitioner care directly within skilled nursing facilities. You will be serving an elderly patient population, residents of long-term care facilities, in a collaborative and supportive clinical environment designed to optimize patient outcomes and streamline care delivery.

We are seeking an autonomous and compassionate clinician with a strong background in adult, family, or geriatric care. The ideal candidate is comfortable managing complex chronic conditions, possesses excellent clinical judgment, and thrives as a collaborative team player within an interdisciplinary healthcare setting.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
