This role involves delivering direct patient care to residents within skilled nursing facilities. You will manage a panel of elderly patients, addressing their primary care needs, chronic conditions, and acute changes in health status. The day-to-day clinical environment is collaborative, working closely with facility staff and attending physicians to ensure seamless, high-quality care for a vulnerable patient population.

We are looking for an autonomous and proactive clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions, possesses excellent clinical judgment, and thrives in a team-oriented setting. Your ability to collaborate effectively with interdisciplinary teams will be crucial in providing holistic care to our residents.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
