FT / PT Wound Nurse Practitioner

This is a unique opportunity for a dedicated Nurse Practitioner to join a compassionate team in Erie, PA, focusing on advanced wound care within a facility-based long-term care model. The ideal candidate is an autonomous clinician with a strong background in geriatric, adult, or family specialties, comfortable managing complex chronic conditions. We seek a collaborative team player who can seamlessly integrate with facility staff and attending physicians to deliver exceptional patient outcomes.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with PA law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in PA (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Wound assessment, NPIAP staging, advanced wound products, wound vac experience, wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
