We are seeking an autonomous clinician with a background in geriatric, adult, or family specialties, comfortable managing complex chronic conditions. The ideal candidate is a collaborative team player, eager to integrate into an interdisciplinary care team and committed to delivering high-quality, patient-centered wound care.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with OH law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in OH (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Strong wound assessment skills, experience with NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
