As a Nurse Practitioner, you will deliver comprehensive care directly within skilled nursing facilities, serving an elderly patient population with diverse and complex needs. The day-to-day clinical environment is dynamic, requiring a proactive approach to managing chronic conditions and acute changes in patient health.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family specialties. The ideal candidate is comfortable managing complex chronic conditions, possesses strong clinical judgment, and thrives as a collaborative member of an interdisciplinary healthcare team.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with KY law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in KY (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Strong wound assessment skills, experience with NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
