Part Time / PRN Wound Care Nurse Practitioner

Our model focuses on providing advanced practice nursing care directly within skilled nursing facilities, ensuring continuity and quality of care for the elderly and long-term care residents. As a Nurse Practitioner, you will manage a diverse patient panel, addressing their primary care needs in a collaborative and integrated clinical environment, reducing the need for frequent hospitalizations and improving patient outcomes.

We are seeking an autonomous and compassionate clinician with a strong background in geriatric, adult, or family medicine. The ideal candidate is comfortable managing complex chronic conditions, possesses excellent clinical judgment, and thrives in a collaborative team environment, working closely with facility staff and attending physicians to optimize patient care.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
