You will operate in a collaborative clinical setting, managing complex cases and ensuring continuity of care for those who need it most.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family specialties. The ideal candidate is comfortable managing complex chronic conditions and excels as a collaborative team player, working alongside facility staff and attending physicians to optimize patient outcomes.

Key Responsibilities

- Provide primary/wound/telehealth care (as appropriate) to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with KY law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in KY (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Demonstrated expertise in wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
