This position involves providing facility-based Nurse Practitioner care within skilled nursing facilities, focusing on the elderly and long-term care resident population. You will operate within a collaborative clinical environment, working closely with facility staff to ensure comprehensive and compassionate care for patients with complex wound care needs.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family specialties. The ideal candidate is comfortable managing complex chronic conditions, particularly those related to wound care, and thrives in a collaborative team setting. Your expertise will be crucial in delivering high-quality, patient-centered care.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Demonstrated expertise in wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
