You will manage a diverse patient panel, addressing their complex health needs in a dynamic, interdisciplinary clinical setting.

We are seeking an autonomous and highly skilled clinician with a background in geriatric, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions, possesses strong clinical judgment, and thrives as a collaborative team player dedicated to enhancing patient outcomes.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with KY law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in KY (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Proficiency in wound assessment, NPIAP staging, and the application of advanced wound products; experience with wound vacs is highly desirable; wound certification preferred.

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
