This is a unique opportunity for a dedicated Nurse Practitioner to provide specialized wound care within a facility-based long-term care model in Osborne, Kansas. In this role, you will deliver comprehensive, facility-based Nurse Practitioner care within skilled nursing facilities, focusing on the elderly and long-term care residents. The day-to-day clinical environment involves collaborating with facility staff to manage and improve patient outcomes in a supportive setting.

We are seeking an autonomous clinician with a background in geriatric, adult, or family specialties. The ideal candidate is comfortable managing complex chronic conditions, possesses strong wound care expertise, and thrives as a collaborative team player within an interdisciplinary healthcare environment.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Wound assessment, NPIAP staging, advanced wound products, wound vac experience, wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
