This position operates within a facility-based model, focusing on providing comprehensive Nurse Practitioner care to residents in skilled nursing facilities. You will primarily serve an elderly patient population, addressing their complex wound care needs within a day-to-day clinical setting that emphasizes continuity of care and interdisciplinary collaboration.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions, particularly those related to wound care, and thrives as a collaborative team player dedicated to improving patient outcomes.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Wound assessment, NPIAP staging, advanced wound products, wound vac experience, wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
