As a facility-based Nurse Practitioner, you will provide expert wound care within skilled nursing facilities, serving an elderly patient population with diverse long-term care needs. The day-to-day clinical environment involves working closely with facility staff to manage and treat complex wounds, ensuring optimal patient outcomes and comfort.

The ideal candidate is an autonomous clinician with a background in geriatric, adult, or family specialties, comfortable managing complex chronic conditions. We are seeking a collaborative team player who can effectively communicate with nursing home staff, attending physicians, and patients' families to ensure integrated and holistic care.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Arkansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Arkansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Wound assessment, NPIAP staging, advanced wound products, wound vac experience, wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
