This position involves delivering facility-based NP care within skilled nursing facilities, catering to an elderly patient population and long-term care residents. The day-to-day clinical environment is collaborative, focusing on proactive health management and acute care interventions for a stable patient base within a structured setting.

We are seeking an autonomous clinician with a background in geriatric, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions and thrives as a collaborative team player within an interdisciplinary healthcare team.

Key Responsibilities

- Provide primary care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Ohio law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Ohio (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
