We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family care, who is adept at managing complex chronic conditions. The ideal candidate thrives in a collaborative environment, working closely with interdisciplinary teams to optimize patient outcomes and enhance the quality of life for our residents.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kansas law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kansas (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Strong skills in wound assessment, NPIAP staging, and application of advanced wound products

- Experience with wound vacs preferred

- Wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
