This is a unique opportunity for a dedicated Wound Nurse Practitioner to make a profound impact on the lives of long-term care residents in a facility-based setting in Somerset, KY. Our model focuses on delivering comprehensive Nurse Practitioner care directly within skilled nursing facilities. You will be serving an elderly patient population, residents of long-term care, managing their complex wound care needs. The day-to-day clinical environment is structured yet dynamic, allowing for focused patient interaction and interdisciplinary team collaboration to ensure optimal resident health.

We are seeking an autonomous and compassionate clinician with a strong background in adult, family, or geriatric specialties. The ideal candidate is comfortable managing complex chronic conditions, particularly those related to wound care, and thrives as a collaborative team player within a facility-based healthcare team.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Experience in wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
