This position involves providing comprehensive nurse practitioner care within skilled nursing facilities, focusing on the elderly and long-term care resident population. You will operate within a structured clinical environment, collaborating with facility staff and attending physicians to ensure optimal patient outcomes and continuity of care for individuals with complex chronic conditions.

We are seeking an autonomous and compassionate clinician with a background in geriatric, adult, or family care. The ideal candidate is comfortable managing complex chronic conditions, possesses strong clinical assessment skills, and thrives in a collaborative, interdisciplinary team setting, contributing to a holistic approach to patient care.

Key Responsibilities

- Provide wound care to long-term care residents in skilled nursing facilities

- Assess and manage acute changes in condition; initiate appropriate diagnostic workup and treatment

- Conduct advanced care planning and manage chronic disease conditions

- Prescribe medications including controlled substances in accordance with Kentucky law

- Communicate with nursing home staff and collaborate with attending physicians on care plans

Qualifications

- Current, unrestricted Nurse Practitioner license in Kentucky (required)

- National board certification (AANP or ANCC)

- Active DEA licensure

- Clinical background in adult, family, or geriatric specialties

- Demonstrated expertise in wound assessment, NPIAP staging, advanced wound products, and wound vac experience; wound certification preferred

- Experience in long-term care or skilled nursing facility setting preferred

- Proficiency with electronic medical records (EMR)
